About Hospital in the Home & Virtual Care
Hospital in the Home & Virtual Care allows the transfer of traditional acute admitted care to the patient's own home without compromising patient care. Care may be provided via in-home nursing and support services, video telehealth, or admission to the Hospital in the Home – Virtual Care, which enables higher acuity care using wearable technology.
About the role
Hospital in the Home (HITH) & Virtual Care (HITH-VC) Case Manager proactively manages patient transfer from traditional inpatient care to high-quality acute care at the patient's home. They collaborate to meet patient needs, ensure high satisfaction, and facilitate efficient discharge planning to community services post HITH admission. Care is delivered through various means such as in-person nursing, Telehealth, virtual ward admission, or other pathways.
A typical day involves:
- Meet patients to assess their needs, collaborate on a care plan, and coordinate services
- Review and manage changes in care needs or clinical deterioration
- Collaborate for effective discharge planning to community care
About you
The ideal candidate will bring:
- Positive attitude to change and innovation
- Collaborative approach to care
- Excellent clinical assessment skills from nursing experience
- Understanding of information technology and willingness to learn new systems
- Postgraduate certification in Cardiac/Critical Care or relevant field
- Previous experience in hospital in the home, case management, or nursing leadership
