About the Role
This is an exciting opportunity to join our team as an Advanced Case Manager, ARC. As a key member of our multidisciplinary team, you will play a crucial role in supporting participants with complex needs, including mental illness, cognitive impairment, and homelessness.
Key Responsibilities
- Provide complex case management support using a holistic and collaborative approach to participants with a range of needs.
- Work closely with the judiciary, legal professionals, and service/court staff to ensure effective case management and support.
- Develop and implement individualized case plans to support participants in achieving their goals and improving their well-being.
- Provide coaching and mentoring to case managers, particularly in areas of advanced knowledge and experience.
- Establish and maintain effective working relationships with stakeholders to ensure seamless case management and support.
About You
We are seeking an experienced and skilled case manager who is passionate about making a difference in the lives of our participants. You will have:
Tertiary qualifications (Diploma level highly desirable) and/or relevant case management experience.Extensive case management expertise to provide client-centered, strengths-based, and solution-focused support.Experience communicating hope and optimism for change and applying a person-centered and evidence-based approach to enhance the client's motivation.Capacity to establish effective working relationships with stakeholders and communicate complex information in a clear and concise manner.How to Apply
