Senior Community Care RN Case Manager

We are seeking a highly skilled and compassionate Registered Nurse to join our Senior Community Care team as a Case Manager. In this role, you will work closely with our interdisciplinary team to provide high-quality care to patients in their homes, nursing homes, and assisted living facilities.
Key Responsibilities- Assist providers with care coordination activities, including benefit determination and therapy coordination
- Collaborate with nursing facilities and primary care teams to identify and respond to patient changes in condition
- Document information and activities in the Electronic Medical Record (EMR)
- Participate in facility-based therapy discussion meetings and family conferences
- Assist with patient care in the state of Florida, either face-to-face or telephonically
- Associate degree or higher in Nursing (RN)
- Current unrestricted Registered Nurse license in the state of Florida
- Certified in Basic Life Support
- Valid driver's license with access to reliable transportation
- 1+ years of experience in post-acute care, such as long-term care
- 1+ years of experience working with the geriatric population
- 1+ years of experience with using an electronic medical records
- Bilingual English/Spanish
