About the Role

We are seeking a highly skilled and compassionate Registered Nurse to join our Senior Community Care team as a Case Manager. In this role, you will work closely with our interdisciplinary team to provide high-quality care to patients in their homes, nursing homes, and assisted living facilities.

Key Responsibilities
  • Assist providers with care coordination activities, including benefit determination and therapy coordination
  • Collaborate with nursing facilities and primary care teams to identify and respond to patient changes in condition
  • Document information and activities in the Electronic Medical Record (EMR)
  • Participate in facility-based therapy discussion meetings and family conferences
  • Assist with patient care in the state of Florida, either face-to-face or telephonically
Requirements
  • Associate degree or higher in Nursing (RN)
  • Current unrestricted Registered Nurse license in the state of Florida
  • Certified in Basic Life Support
  • Valid driver's license with access to reliable transportation
Preferred Qualifications
  • 1+ years of experience in post-acute care, such as long-term care
  • 1+ years of experience working with the geriatric population
  • 1+ years of experience with using an electronic medical records
  • Bilingual English/Spanish
