Job Summary

The Clinical Manager is a key member of our team at Pinnacle Home Care Inc., responsible for ensuring that patient care is coordinated and managed effectively. This role requires a strong commitment to delivering high-quality care to our patients in their homes.

Key Responsibilities
  • Receives case referrals and reviews available patient information to determine home care needs.
  • Assigns appropriate clinicians to cases and reviews and evaluates each case to ensure quality care is delivered.
  • Reviews patient clinical diagnosis, medications, procedures, and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals and developing the plan of care.
  • Attends case conference meetings to facilitate coordination of care.
  • Assists in the screening and interviewing process of new personnel and makes recommendations for employment.
  • Complies with accepted professional standards and principles.
  • Consistently follows and enforces Agency policies and procedures.
  • Leads team by Agency Mission, Vision, and Values and drives operations aligned with our Sacred Seven.
  • Assesses clinicians on an ongoing basis to ascertain their understanding and compliance with policies and procedures.
  • Participates in public relations and community activities that promote the Organization's role as an effective member of the health care delivery system.
  • Performs consistent 1:1 meetings with clinicians, providing monthly SHP scorecards and continued education.
  • Performs field visits with clinicians to mentor and support clinical excellence.
  • Promotes the use of the patient portal, physician portal, and Synzi.
  • Promotes Telehealth visits to support appropriate visit utilization.
  • Performs other duties and activities as delegated by the Director of Clinical Services.
Qualifications
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing or accredited school of nursing. BSN degree is preferred.
  • Two years of management or supervisory experience in a health care setting, preferably home care.
  • Complies with accepted professional standards and practice.
  • Complies and maintains current CPR certification.
  • Demonstrates excellent observation, organization, and verbal and written communication skills.
  • Proficient in Computers, Microsoft, EMR, and Technology.
  • Knowledge of governmental regulations and ACHC standards.
  • This position requires that the employee is willing and able to meet the physical requirements as necessary.
