Job Summary

The Clinical Manager is a key member of our team at Pinnacle Home Care Inc., responsible for ensuring that patient care is coordinated and managed effectively. This role requires a strong commitment to delivering high-quality care to our patients in their homes.

Key Responsibilities
  • Receive case referrals and review patient information to determine home care needs.
  • Assign appropriate clinicians to cases and review services provided to ensure quality care.
  • Review patient clinical diagnosis, medications, procedures, and clinical course.
  • Assist clinicians in establishing therapeutic goals, setting priorities, and developing plans of care.
  • Attend case conference meetings to facilitate coordination of care.
  • Screen and interview new personnel, making recommendations for employment and assisting with their orientation.
  • Comply with professional standards and principles, and consistently follow and enforce Agency policies and procedures.
  • Lead the team by Agency Mission, Vision, and Values, and drive operations aligned with our Sacred Seven.
  • Assess clinicians on an ongoing basis to ensure understanding and compliance with policies and procedures.
  • Participate in public relations and community activities that promote the Organization's role in the healthcare delivery system.
  • Perform 1:1 meetings with clinicians, providing monthly SHP scorecards and continued education.
  • Perform field visits with clinicians to mentor and support clinical excellence.
  • Promote the use of patient and physician portals, and Synzi.
  • Promote Telehealth visits to support appropriate visit utilization.
  • Perform other duties and activities as delegated by the Director of Clinical Services.
Qualifications
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Graduate of National League for Nursing or accredited school of nursing. BSN degree is preferred.
  • Two years of management or supervisory experience in a healthcare setting, preferably home care.
  • Comply with accepted professional standards and practice.
  • Comply and maintain current CPR certification.
  • Demonstrate excellent observation, organization, and verbal and written communication skills.
  • Proficient in Computers, Microsoft, EMR, and Technology.
  • Knowledge of governmental regulations and ACHC standards.
  • This position requires that the employee is willing and able to meet the physical requirements as necessary.
