AAW Supports Coordinator


Location:
(Hybrid/Remote with 50% travel) Philadelphia and surrounding Areas within the state of PA


Type:
Full-time employee or contractor


Responsibilities:

  • Develops an individualized plan of service (ISP) initially and annually
  • Completes required assessments initially and annually
  • Acts as the Chairperson of the multidisciplinary team
  • Conducts Quarterly in person monitoring
    (every 3 months) 4 Times per year, per consumer:
  • Collecting and reviewing provider monthlies and completes Quarterly monitoring using provider monthlies
  • Monitor all services provided on behalf of client weekly, monthly, and quarterly
  • Monitor satisfaction with services
  • Submit Critical Revisions to ISP to add, update or changes services
  • Develop and maintain computerized Support Coordinator notes for records necessary for professional practice as defined in the procedural manual
  • Weekly or as need team meeting(s) with Supports Coordinator Supervisor and/or other team members
  • As needed Provide outreach services to individuals and their families in their respective locales and/or homes as appropriate to their function
  • As needed Provide community education services as assigned
  • As needed Participate in clinical supervision, team meetings and case consultations
  • As needed Obtain all prior authorizations and applicable coordination of care
  • AS needed Provide resource development in an attempt to reduce the amount of service gaps
  • Follow Helping Hands Community Services and state level professional code of conduct
  • Represent Helping Hands Community Services in the community
  • Perform other duties as assigned
  • Completion of SC notes in electronic system and SC Job Duty document for weekly pay.

Requirements:

Must meet the following minimum educational and experience requirements:


  • A bachelor's degree, which includes or is supplemented by at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science; or:
  • Two years' experience as a County Social Service Aide 3 and two years of college level course work, which includes at least 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social service; or

Any combination of experience and training which includes 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social service and one year of experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions; or:

  • A combination of 12 college credits in sociology, social work, psychology, gerontology, criminal justice, or other related social science and two years professional experience in developmental disabilities, special education, mental health, counseling psychology, school psychology, social work or health and rehabilitative services.2. Have criminal history clearances per 35 P.S et seq. and 6 Pa. Code Chapter 15.
  • Have a valid driver's license if the operation of a vehicle is necessary to provide Support Coordination services.4. Newly hired Supports Coordinators must successfully complete required Supports Coordination Orientation Curriculum.5. Complete a minimum of 24 hours of training a year.
