Position Overview:

The Case Manager Supervisor is responsible for coordinating case management, training new case managers, and supervising the work of other case managers. The Case Manager Supervisor will provide field-based and in-office case management services to guests. The Case Manager Supervisor is responsible for coordinating regular meetings to identify and assess the guests' needs and facilitating access to appropriate resources and supportive services. The Case Manager Supervisor will be accountable, in the absence of the assigned case manager, for developing and maintaining positive collaborative relationships with residents, colleagues, community partners, and other service providers to promote participant engagement and continuity of care. The ongoing responsibilities include supportive interventions for care retention, care coordination, quarterly assessments and weekly documentation of residents' progression towards completion of set goals. The Case Manager Supervisor is expected to provide structured case management services consistent with harm reduction principles.?

Responsibilities:

  • Provide daily oversight to the case management team to include, but not limited to, guidance related to service delivery and documentation, maintaining compliance with regulatory standards, appropriately managing caseloads, and coordinating on-call scheduling.
  • Effectively self-manage and collaborate with all levels of staff and clients with varying needs ensuring timely and efficient utilization of resources; preventing return to homelessness and ensuring adherence to organization and regulatory requirements; ensuring licensure compliance as well as business process improvement measures for quality assurance and self-development as well as development of the organization.
  • Participate in weekly staff meetings, scheduled supervision meetings, and other required meetings and trainings.
  • Proactively formulate and manage outreach activities for caseload to promote resident interest.
  • Work with existing field staff to identify residents that would benefit from personalized support
  • Identify residents who are interested in case management services and those that would benefit from information to supportive services.
  • Educate residents on services of interest and need, treatment courses, risks, and health outcomes of certain behaviors.
  • At time of screening/intake assessment, the Case Manager will address emergent/immediate needs of client including need for medical care, shelter, food and clothing, and harm reduction services and supplies.
  • In collaboration with residents, develop and implement an Individual Service Plan (ISP); the ISP shall address client needs, risk factors, barriers, goals, and interventions to meet those goals. Goals may include shelter, housing, food, clothing, health care, substance use treatment, harm reduction strategies, mental health care, education/training, employment resources, etc.
  • Update ISPs periodically to reflect resident's progress towards or attainment of articulated goals and the emergence of client needs; ISP's shall assist the client in developing their own autonomy.
  • Assist residents in accessing appropriate community resources that represent the client's needs based on assessment or clients' verbal input.
  • Distribute safe drug use supplies and overdose prevention kits when necessary.
  • Develop and maintain positive, collaborative relationships with community partners and other service providers including substance use treatment providers, mental health providers, housing resources, and primary healthcare.
  • Develop and maintain client charts with appropriate and up to date documentation.
  • Provide dependable and structured Case Management Services consistent with harm reduction principles utilizing a whole person approach.
  • Assist residents with enrollments and retention with other service programs, as well as scheduling and keeping appointments.
  • Identify gaps and barriers in available community resources and advocate for systemic changes.
  • Attend Case Management staff meetings and other regularly required meetings.
  • Participate in regular upkeep, cleaning, stocking, and supervision of Drop-in area one time weekly or as needed.
  • Take the lead position in communication with each member of the participant's case management team of service providers and caregivers by coordinating care with other providers, and caregivers and by convening case conferences to ensure service continuity and elimination of gaps in service.
  • Provide intensive and restorative care management support.
  • Other duties as assigned.

Education:

Bachelors' degree from an accredited institution

Master's degree and licensed-eligible preferred

Experience:

  • Must have supervisory experience with a team and/or project management.
  • Must have at least 3 years of demonstrated knowledge and experience in the field of direct services to individuals with low-income, behavioral health, and/or history of homelessness; establishing supportive trusting relationships with such clients, peers, and key partners, respecting rights and personal preferences.
  • Understanding of substance use and harm reduction strategies preferred.
  • A demonstrated passion for serving chronically homeless populations with co-occurring disorders.
  • Flexible team player who takes initiative
  • Ability to set boundaries, resolve conflict, and de-escalate unexpected issues.
  • Ability to work independently and seek supervision when appropriate.
  • Ability to prioritize workload and complete tasks in a timely and efficient manner.
  • Must exhibit proven success working with change management and culture shifts, exhibiting day-to-day flexibility in handling multiple priorities and organization initiatives, maintaining accountability for achieving improved performance metrics ongoing while exhibiting a positive attitude even when challenging.
  • Dependable, able to work under pressure; receptive to change, willingness to learn, cooperative approach to problem-solving.
  • Computer literate, with basic knowledge of Microsoft Office, as well as a high level of initiative in keeping current with technological change.
  • Good basic clerical skills, including typing, ability to proofread own work, good grammar and spelling
  • Ability to read and interpret general business correspondence, policies and procedures, referral information, financial information, financial documentation, applicable government regulations.
  • Ability to write case notes, uncomplicated reports, instructions, and procedures.
  • Ability to present information effectively and respond to questions from residents, referral sources and the general public.
  • Record keeping and confidentiality.
  • All information seen/heard at our locations is CONFIDENTIAL.
  • This includes the site name and address, and any information about guests or staff.
  • Demonstrates a strong understanding of the city and state political and community service environment (e.g., community service leaders, partners, organizations, critical state-wide community service needs, legislative leaders, government officials, major stakeholders, etc.).?
  • Previous government relations, non-profit and grassroots organizing experience strongly preferred.?
  • Non-Profit experience working in local or state government preferred.
