Position Summary: We are seeking a dedicated and experienced Case Manager Supervisor to join our team. The ideal candidate will have a passion for supporting both clients and Case Managers, ensuring high-quality care and effective case management. This role involves a blend of assessment, training, support, and direct case management duties.

Core Duties and Responsibilities:

Assessments:

  • Develop appropriate assessment tools to measure client outcomes.
  • Conduct research to identify pre-structured assessments suitable for our clientele.
Case Manager Onboarding Training:
  • Train newly onboarded Case Managers on various tasks including:
    • Google Docs & documentation
    • Assessments & treatment plans
    • New client paperwork & Release of Information (ROIs)
    • PayChex procedures
    • Camden Patient Portal & Zoom usage
    • Weekly financial sheet management
Case Manager Education Curriculum:
  • Meet with external trainers to organize and structure training sessions effectively.
  • Admit Case Managers into the Shadow Program and monitor progress.
Case Manager Fill-In:
  • Fill in as a Case Manager when necessary, ensuring continuity of care for clients.
Case Manager Support and Advocate:
  • Conduct weekly supervision with Case Managers to offer guidance on cases.
  • Provide resources and assistance to address any challenges faced by Case Managers.
Case Manager Training Manual:
  • Collaborate with the Director of Operations (DOO) to develop a comprehensive training manual and accompanying videos for new Case Managers.
  • Ensure that training materials are up-to-date and easily accessible to all staff members.
Family Communication:
  • Communicate with families on clinical progress and case management satisfaction.
Camden App Communication with Case Managers:
  • Keep up with the communications of all team threads and answer questions from Case Managers as needed or manage crises out of their scope.
Admissions
  • Assist the admissions team by conducting intake assessments for potential clients, ensuring accurate and thorough evaluation of client needs.
Qualifications:
  • Master's degree in Social Work or a related field.
  • Preferred: Valid LCSW (Licensed Clinical Social Worker)
  • Proven experience in case management and social work.
  • Strong organizational and training skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with a team and manage multiple tasks effectively.
