Intensive Case Management Health Caseworker

- Collaborate closely with the ICM team to identify additional healthcare needs for vulnerable clients.
- Assist clients in navigating healthcare systems, which may include attend and schedule appointments.
- Coordinate health plan selection and advocate to healthcare centers on behalf of clients who are having difficulty accessing services.
- Develop and distribute healthcare guides tailored to each client.
- Assist clients with the selection of a healthcare specialist and help schedule initial appointments once the selection has been made, ensuring each client is comfortable with contacting their specialist and with scheduling their own appointments.
- Conduct regular outreach to healthcare providers and social service agencies to address health needs and identify resources.
- Provide clients with extended orientation on a variety of topics regarding health such as womenâs health, specialized treatment for people with physical disabilities, and navigating physical healthcare systems at the local level.
- Respond to client health-related crises, with guidance from supervisor as needed.
- Maintain up-to-date documentation in clientâs case file to ensure services are properly documented throughout the clientâs service period; document all services provided in IRCâs database, Efforts to Outcomes (ETO).
- Attend relevant trainings and meetings at the local and national level.
- May train and lead the activities of volunteers and interns.
- Promote and actively participate in initiatives and efforts to build team engagement, inclusion and cohesion in IRC San Jose
- Foster ongoing learning, honest dialogue and reflection to strengthen safeguarding and to promote IRC values and adherence to IRC policies
- Other duties as assigned.
- Strong relationship building, diplomacy, and networking skills; ability to effectively build internal and external relationships.
- Demonstrated success working and communicating effectively in a multi-cultural environment.
- Self-starter with excellent problem-solving skills combined with the proven ability to prioritize and manage time effectively.
- Attention to detail and accuracy in work product.
- Fluent in English required, both spoken and written; proficiency in Dari, and Pashto is desired.
- Proficient in Microsoft Office applications (Word, Excel, Outlook); ability to use the internet and other digital tools for data entry, research, and problem-solving purposes.
- Valid driverâs license and access to a personal, insured vehicle.
- A combination of standard office environment, remote work, and âfieldâ time within the service delivery area to perform the above outlined responsibilities.
- May require occasional weekend and/or evening work.
Posted pay ranges apply to US-based candidates. Ranges are based on various factors including the labor market, job type, internal equity, and budget. Exact offers are calibrated by work location, individual candidate experience and skills relative to the defined job requirements.

US Benefits: The IRC offers a comprehensive and highly competitive set of benefits.
