WHAT DO YOU DO AS A YOUNG ADULT LIFESET SPECIALIST

This is a full-time position. The Young Adult LifeSet Specialist will need to maintain a flexible schedule to accommodate the needs of the young adults, including participating in a 24/7 on-call rotation for after-hours, weekends, and holidays.

As a Young Adult LifeSet Specialist, your day will be dedicated to providing individualized services to 6 - 8 young adults in their homes and community settings. You will meet with each young person at least once a week face-to-face, with additional support provided via phone, text, or in-person as necessary. Some days may require daily visits to ensure safety and stability. You will be directly responsible for all interventions, collaborating with key stakeholders such as probation officers, child welfare workers, and therapists to support the young adults and their families. Your role includes conducting ongoing assessments from a strength-focused, solution-based perspective, developing monthly service plans, and documenting all interactions within organizational timelines. You will attend various meetings, from court hearings to IEP meetings, always advocating for the long-term stability of the young adults. Participation in team supervision sessions and training is expected, as well as implementing feedback to continually enhance your practice. Additionally, you will conduct detailed case reviews to identify and engage significant adults in the youth's life, coordinating visiting plans and ensuring compliance with all DCFS requirements.


DO YOU HAVE WHAT IT TAKES?

  • Bachelor's degree in social services with two years of related experience in counseling or case management
  • Licensure by DCFS or eligible for licensing
  • Valid driver's license, proof of insurance, and reliable transportation
  • Strong organizational skills and attention to detail
  • Ability to maintain a flexible schedule and to manage multiple priorities simultaneously
  • Strong communication skills

A master's degree in the field of social work, psychology, or related field is preferred but not required. Knowledge or experience working with adolescents is also preferred.
