Position Overview:

This role is designed for a dedicated Licensed Practical Nurse (LPN) who will serve as a Care Coordinator, ensuring seamless patient navigation and oversight for participants in the Medically Complex Children's Waiver program.

Key Responsibilities:

  • Manage a caseload of clients within a designated area, collaborating closely with the assigned RN case manager to address the needs of clients in their home environments.
  • Conduct thorough information gathering through both in-person visits and telephonic communication.
  • Implement service plans and care strategies developed by the RN case manager, ensuring comprehensive support for each client.
  • Regularly assess existing services and identify any new needs or barriers to care, facilitating timely adjustments to care plans.
  • Perform biannual on-site visits with clients and their families to review and update care plans, ensuring that all services provided are effective and relevant.
  • Engage in collaborative case conferences with the RN case manager to discuss new needs and potential updates to service plans.
  • Participate in monthly staff meetings and case conferences, contributing to a team-oriented approach to client care.
  • Manage travel and transportation expenses associated with client visits.

Qualifications:

  • Must possess a valid LPN license in accordance with state regulations.
  • Preferred experience in Medicaid programs and bilingual capabilities.
  • Minimum of three years of experience in Pediatric Nursing, with a strong preference for Pediatric Home Care experience.
  • Graduate of an accredited LPN program.
  • Ability to travel locally with a valid driver's license and automobile insurance.

Technical Skills:

Proficiency in software applications such as MS Word, PowerPoint, Excel, Outlook, and Google Suite is essential. The role involves accessing, inputting, and retrieving information from computer systems, often while engaging in telephonic communication.

Work is primarily conducted in an office environment, with potential repetitive motions associated with data entry and monitoring visual information.
