Position Overview

The main objective of this role is to deliver, manage, and oversee hospice care services in accordance with physician directives and the patient’s care plan, while adhering to hospice policies and protocols through the application of nursing principles.

Reporting Structure

Reports to: Area Clinical Manager
Collaborates with: Interdisciplinary Team (IDT) and administrative personnel

Key Responsibilities

  • Embrace the mission of Patriot Hospice, emphasizing the core values of human dignity and a compassionate response to the needs of individuals facing life-limiting or chronic illnesses.
  • Actively participate as a member of the Interdisciplinary Team (IDT) and attend relevant meetings.
  • Adhere to the Nursing Standards set forth by Patriot Hospice.
  • Maintain knowledge of hospice policies, as well as state and federal regulations related to hospice and nursing services.
  • Implement nursing care in compliance with current regulations and guidelines.
  • Provide written and verbal reports to the Area Clinical Manager regarding the nursing service department's operations as needed.
  • Engage in the hospice quality assurance program as directed by the Area Clinical Manager.
  • Foster effective relationships with nursing staff at contracted facilities to ensure seamless integration of hospice services into the overall care plan for patients.
  • Ensure thorough documentation of patient care services to support accurate billing.
  • Perform administrative tasks such as completing medical forms, reports, evaluations, and charting as required.
  • Oversee the care provided by Hospice Aides.
  • Manage the schedules of Certified Nursing Assistants (CNA).
  • Report any occupational exposures to blood, body fluids, or hazardous materials following the hospice's policies.

Nursing Care Responsibilities

  • Assume responsibility for coordinating the physical care of patients by educating primary caregivers, volunteers, and family members as necessary.
  • Keep accurate and up-to-date patient records that reflect problems, plans, actions, and goals.
  • Notify hospice and facility staff of any unusual or potentially concerning patient or family issues.
  • Provide appropriate support during the time of death.
  • Supply the Area Clinical Manager with insights regarding the nursing needs of patients.
  • Initiate communication with hospice physicians, attending physicians, other hospice staff, facility personnel, and external agencies to coordinate care and resources for patients and families.
  • Maintain regular communication with facility nursing staff to review patient care, including participation in care plan meetings.
  • Delegate personal care tasks to Hospice Aides according to the established care plan.
  • Act as a liaison among patients, their families, and support departments to effectively plan for patient needs.

Qualifications

A minimum of a Nursing Degree from an accredited institution and a valid Registered Nurse license in the State of South Carolina is required. A year of nursing experience in a hospital, long-term care facility, or similar healthcare setting is preferred, with hospice experience being advantageous.

Knowledge, Skills, and Abilities

  • Possession of a current, unencumbered Registered Nurse license in the state.
  • Ability to read, write, speak, and comprehend the English language.
  • Capacity to make independent decisions when necessary.
  • Ability to interact tactfully with personnel, patients, families, visitors, and government agencies.
  • Knowledge of hospice nursing practices, medical procedures, and relevant laws and regulations.
  • Demonstrated leadership and supervisory skills, with a collaborative approach to working with others.
  • Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for quality care delivery.
  • Patience and tact, especially when dealing with challenging situations.
  • Ability to communicate effectively regarding patient conditions.
  • Commitment to maintaining a safe workplace environment.
  • Proficient computer skills for documentation purposes.
  • Understanding of HIPAA compliance and regulations.

Working Environment

  • Travel within the service area using a personal vehicle is required; must possess a valid driver’s license and auto insurance.
  • Work occurs in office settings, community environments, and long-term care facilities.
  • Intermittent movement during working hours is expected.
  • Frequent interruptions may occur.
  • Engagement with patients, personnel, visitors, and government agencies under various conditions is part of the role.
  • May encounter emotionally charged situations with patients and families.
  • Collaboration with medical staff, nursing personnel, and department supervisors is essential.
  • Availability for work beyond normal hours, including weekends and holidays, may be necessary, along with on-call responsibilities.
  • Participation in community health initiatives and continuing education programs is encouraged.
  • Exposure to potential hazards, including infectious materials and environmental conditions, is a possibility.
  • Physical demands include the ability to lift and move patients or equipment as necessary.
