Role Overview

We are seeking a skilled Care Manager RN to join our team at Swedish Cherry Hill. As a key member of our care management team, you will play a vital role in coordinating patient care and services across the acute care episode.

Responsibilities

Your primary responsibilities will include:

  • Coordinating the care and service of selected patient populations through the acute care episode, across the continuum.
  • Working collaboratively with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources.
Requirements

To be successful in this role, you will need:

  • A Bachelor's Degree in Nursing or related field.
  • Washington Registered Nurse License upon hire.
  • 3 years Registered nursing experience in an acute care hospital.
Preferred Qualifications

Prior experience in hospital case management is highly desirable.
