Job Overview

Position Title: Patient Navigator

Department: Rehabilitation Medicine

Contract Duration: 4 Months

Working Hours: 10:00 am - 6:00 pm (includes a 30-minute break)

Position Summary: The Patient Navigator acts as a vital link between patients and the healthcare team, ensuring an effective and seamless patient journey within the Rehabilitation Medicine department. This role encompasses assisting with patient check-ins, addressing patient inquiries, and coordinating various facets of patient care.

Key Responsibilities:

  • Facilitate patient check-ins, ensuring all required documentation is completed accurately.
  • Address patient requests and inquiries in a professional and timely manner.
  • Collaborate with healthcare providers and administrative personnel to enhance patient care.
  • Maintain and update patient records, ensuring adherence to healthcare regulations and confidentiality standards.
  • Support patients and their families in navigating the healthcare system and comprehending their treatment plans.
  • Perform general administrative tasks, including data entry, filing, and appointment scheduling.
  • Oversee and manage patient flow within the department to maximize efficiency and patient satisfaction.

Qualifications:

  • 1-3 years of experience in administrative roles within a medical environment.
  • Proficient in basic computer skills, particularly Microsoft Word and Excel.
  • Familiarity with EPIC or similar electronic medical record systems.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with patients, families, and healthcare professionals.
  • Capable of working independently and managing multiple tasks in a dynamic environment.
  • Commitment to maintaining confidentiality and privacy standards in handling patient information.

Education Requirements: High School Diploma or GED is required; additional education in healthcare administration or a related field is preferred.
