Key Responsibilities:
- Patient Management:
- Perform detailed evaluations, create individualized care strategies, and conduct regular follow-up assessments for residents in rehabilitation and extended care facilities.
- Oversee a range of health issues, including but not limited to diabetes, hypertension, respiratory conditions, and neurological ailments.
- Authorize medications and therapies, modifying them as necessary based on patient feedback.
- Interdisciplinary Collaboration:
- Work closely with physicians, nursing personnel, physical rehabilitation specialists, and other healthcare providers to synchronize patient care.
- Engage in team discussions to review patient advancements and treatment methodologies.
- Documentation and Regulatory Compliance:
- Ensure precise medical record-keeping and adherence to healthcare regulations and institutional policies.
- Timely completion of documentation, including progress reports and discharge summaries.
- Patient and Family Education:

Qualifications:
- Current licensure as a Nurse Practitioner or Physician Assistant.
- Certification from an accredited NP or PA program.
