Job Summary

We are seeking a highly skilled and experienced Care Coordination Supervisor to join our team at Multnomah County. As a key member of our Behavioral Health team, you will be responsible for supervising the Substance Use Assessment Team, Jail Coordination Team, and a Housing Support Specialist. Key Responsibilities
  • Supervise and lead a team of professionals in providing high-quality care coordination services to individuals and families.
  • Develop and implement program plans and strategies to achieve departmental goals and objectives.
  • Collaborate with program leadership to develop and implement new programs and services.
  • Provide administrative and clinical supervision to team members, ensuring quality of care and compliance with county and departmental policies and procedures.
  • Research and gather program, local, state, federal, and funder-based rules, policies, procedures, and requirements to apply to program functions, training of staff, application to team licenses/certifications, and application in compliance with county collective bargaining agreements, Personnel Rules, etc.
  • Collaborate with program and division leadership to communicate and resolve any conflicts or undefined domain, to clarify to limit program liability and safety concerns.
  • Participate in ongoing administrative meetings with multiple county programs, community organizations, and State personnel for the purpose of program review and planning, quality improvement and assurance activities.
  • Develop and help implement strategic community outreach plans.
  • Oversee adherence to federal, state, county, and division regulations and rules and best practices for clinical documentation and recordkeeping.
  • Provide ongoing training to staff to enhance clinical expertise.
  • Oversee hiring, orientation, and performance evaluations for new and existing staff, including disciplinary action when needed.
  • Support ongoing performance development through coaching, meetings, and career planning.
  • Develop and organize staff training to ensure quality assurance, improvement, and adherence to best practices.
  • Monitor service delivery and respond to complaints, leading with equity and maintaining confidentiality.
Requirements
  • Certification: Must meet the standards of Certified Alcohol and Drug Counselor - II (CADC-II)
  • Education/Experience: Bachelor's degree from an accredited college or university in the mental health field with major coursework in social science, or a related field AND Three (3) to six (6) years of experience that demonstrates the ability to perform the duties of the position.
  • Preferred Qualifications/Transferable Skills: Leads with race through actions and advocacy with internal programs, with community partners and across departments throughout the county.
