Job Title: Resident Care Coordinator

We are seeking a compassionate and experienced Resident Care Coordinator to join our team at The Holgate Center. As a key member of our nursing staff, you will be responsible for coordinating and supervising the care of our residents in both skilled and long-term care settings.

Key Responsibilities:
  • Coordinate and supervise the care of residents, ensuring that their needs are met and their rights are respected.
  • Supervise and train nursing staff to ensure that they are providing high-quality care and adhering to facility policies and procedures.
  • Participate in the development and implementation of care plans, ensuring that they are individualized and meet the unique needs of each resident.
  • Collaborate with other members of the healthcare team to ensure that residents receive comprehensive and coordinated care.
  • Monitor and report any changes in resident condition, ensuring that appropriate interventions are implemented to maintain their health and well-being.
Requirements:
  • Two years of experience as a supervisor in a hospital, nursing home, or other related healthcare facility.
  • Knowledge of nursing and medical practices and procedures, as well as laws and regulations pertaining to long-term care.
  • Ability to communicate effectively with residents, family members, and healthcare staff.
  • Ability to work independently and as part of a team.
  • Ability to maintain confidentiality and handle sensitive information.
