About the Role

United Homecare Services, a leading non-profit home care agency, is seeking a highly skilled Care Coordinator Assistant to join our team in Beaverton, Oregon. As a key member of our care coordination team, you will play a vital role in ensuring the delivery of exceptional care to our clients.

Key Responsibilities
  • Assist the Care Coordinator in managing client and caregiver schedules, ensuring seamless coordination of care services.
  • Provide exceptional customer service, responding to client inquiries and providing timely information via phone, email, and in-person interactions.
  • Collaborate with caregivers to ensure they have the necessary skills and support to provide high-quality care to clients.
  • Facilitate ongoing caregiver training and development to enhance their skills and knowledge.
  • Evaluate client service plans and update as needed to ensure optimal care outcomes.
  • Be available by phone 24/7 while on-call and be prepared to respond to emergencies with short notice.
  • Travel within our 60-mile service area to perform duties, with additional mileage compensation provided.
  • Maintain accurate and up-to-date records, including electronic data, client files, and employee files.
  • Perform other office duties as needed to support the care coordination team.
