Job Title: Referral Coordinator: Onsite Portland Metro South

As a Referral Coordinator: Onsite Portland Metro South, you will play a critical role in facilitating new patients' transition to our services, ensuring a seamless onboarding experience for patients and referral partners.

Job Summary:

This role is responsible for coordinating discharge and securing nursing and delivery services for new referrals. You will create and communicate required documentation, secure upfront payment, and support ongoing activities to ensure clean claims and denial management.

Responsibilities:
  • Engage with patients, referral sources, sales, pharmacy, and nursing to coordinate discharge and secure nursing and delivery services.
  • Create and communicate required documentation for new referrals.
  • Secure upfront payment, including assisting patients to find avenues for payment where needed.
  • Support ongoing activities to ensure clean claims and denial management.
  • Assist Patient Registration Supervisor and Manager with special assignments as needed.
Requirements:
  • High school diploma or equivalent required.
  • Minimum of two years of experience in related experience required.
  • Ability to multi-task and support numerous referrals/priorities at one time.
  • Ability to work in a fast-paced environment.
  • Must be detail-oriented and have a high degree of quality focus.
  • High degree of customer service skills required.
  • Ability to trouble shoot, problem solve, and collaborate with cross-functional team members.
