Position Overview

This role is for a Clinical Medication Management Pharmacist dedicated to enhancing the Weight Management Health Service line. The pharmacist will operate both onsite and remotely, providing thorough medication management services. This includes assessing prescribed medications, over-the-counter products, herbal supplements, and immunizations to ensure safe and effective use tailored to each patient's unique needs.

Key Responsibilities

  • Deliver comprehensive Medication Therapy Management in accordance with established practice models.
  • Ensure all medications are indicated, effective, safe, and convenient for the patient.
  • Gather patient histories and integrate this information with other clinical data for evaluations.
  • Identify, address, and prevent medication-related issues.
  • Assess clinical signs and symptoms in relation to disease processes and make recommendations or adjustments to medication therapy based on collaborative agreements.
  • Evaluate the significance of adverse reactions and complications related to drug therapy.
  • Communicate therapy goals clearly to patients and ensure collaboration with healthcare providers to achieve these objectives.
  • Document patient outcomes at each encounter as required by the practice model.
  • Maintain professional knowledge and skills through literature review and participation in educational events.
  • Engage in departmental planning and goal-setting activities.
  • Provide mentorship and training to pharmacy students and residents as needed.
  • Maintain active preceptor status as required.

Qualifications

Education:

  • B.S. Degree in Pharmacy or PharmD from an accredited institution.

Licensure/Certification:

  • Current pharmacy license in Minnesota.
  • Certification in Pharmaceutical Care as recognized by Fairview Pharmacy Services Management.
  • Basic Life Support Certification.

Experience:

  • PGY1 residency or a minimum of three years of experience as a practicing pharmacist in a clinical environment.
  • Preferred experience includes working with specialty populations or completing a residency with an ambulatory focus.
