The Minnesota Visiting Nurse Agency is seeking a Pharmacist to be a vital part of our Inpatient Pharmacy division. This part-time role encompasses 16 hours per pay period, with a rotating schedule of day and evening shifts, including every fourth weekend.
Purpose of the Role: The Pharmacist is responsible for the accurate filling and dispensing of medication orders, ensuring the proper handling, usage, and storage of pharmaceuticals within the pharmacy. This position upholds departmental standards and translates policies into daily operations. The Pharmacist serves as a key information resource for patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related inquiries. Additionally, the role involves planning, organizing, and supervising the work of pharmacy personnel as needed.
KEY RESPONSIBILITIES:
- Verify, prepare, compound, and dispense medications accurately.
- Interpret physician medication orders for accuracy, addressing any errors or discrepancies, and recommend appropriate drug therapies based on patient evaluations.
- Maintain precise patient medication profiles, ensuring the correct selection of drugs, dosages, and monitoring for potential interactions.
- Monitor patient responses to drug therapies.
- Oversee the dispensing and record-keeping of controlled substances.
- Act as an information resource for medical staff.
- Counsel patients regarding their prescriptions.
- Stay informed of legal requirements and departmental policies regarding drug distribution, as well as recent advancements in pharmacy.
- Assume leadership responsibilities as needed, coordinating the activities of pharmacists and support staff.
- Conduct educational sessions for medical staff and healthcare personnel about medications and pharmacy procedures.
- Support pharmacy management in aligning departmental goals with organizational objectives.
- Provide ongoing assessment of patient pharmacotherapy needs, collaborating with medical and nursing staff to optimize drug therapy.
- Critically evaluate medical information to provide accurate guidance to physicians.
- Assist in training pharmacy technicians and interns.
- May mentor pharmacy students or residents.
- Perform related duties as required.
QUALIFICATIONS:
- Graduation from an accredited pharmacy program.
Preferred Qualifications:
- Advanced degree or current enrollment in an advanced degree program.
- At least one year of experience as a registered pharmacist in a hospital environment.
- Completion of an ASHP accredited residency.
- Board certification or advanced credentials in a specialized area of practice.
- Completion of ASHP PGY-2 residency in specialized fields (e.g., critical care, infectious diseases).
SKILLS AND ABILITIES:
- Strong problem-solving and communication skills, both verbal and written.
- Self-motivated with the ability to work under pressure and adapt to changing priorities.
- Capable of performing detailed analysis of complex information.
- Exceptional customer service orientation.
- Ability to collaborate effectively with team members and other departments.
LICENSE AND CERTIFICATIONS:
- Valid pharmacy license to practice in the State of Minnesota.
