Position Overview

This role is for a Clinical Medication Management Pharmacist dedicated to supporting the Weight Management Health Service line. The pharmacist will operate both onsite and remotely, providing comprehensive medication management services. This includes evaluating prescribed medications, over-the-counter drugs, herbal supplements, and immunizations to ensure safe and effective use tailored to each patient's unique needs.

Key Responsibilities

  • Deliver Medication Therapy Management in accordance with established practice models.
  • Ensure that all medications are appropriately indicated, effective, safe, and convenient for patients.
  • Gather patient histories and utilize this information alongside other clinical data for thorough evaluations.
  • Identify, address, and prevent medication therapy issues.
  • Assess signs and symptoms related to various health conditions and make necessary recommendations or adjustments to medication therapy based on collaborative practice agreements.
  • Evaluate the significance of adverse reactions and complications associated with drug therapies.
  • Communicate specific therapy goals to patients, ensuring understanding and collaboration with healthcare providers.
  • Document patient outcomes at each encounter as required by the practice model.
  • Maintain proficiency in pharmacy practices through ongoing education and participation in relevant seminars and conferences.
  • Engage in departmental planning and goal-setting initiatives, contributing to the implementation of these objectives.
  • Provide mentorship and training to pharmacy students and residents, ensuring a high-quality experiential learning environment.
  • Maintain active preceptor status as required by regulatory bodies.

  • Stay informed about and adhere to all relevant laws, regulations, policies, and standards.
  • Promote a culture of continuous improvement and innovative thinking.
  • Perform additional duties as assigned.

Qualifications

Education:

  • B.S. Degree in Pharmacy or PharmD from an accredited institution.

Licensure/Certification:

  • Current pharmacy license in Minnesota.
  • Certification in Pharmaceutical Care as recognized by Fairview Pharmacy Services Management.
  • Basic Life Support Certification.

Experience:

  • PGY1 residency or a minimum of three years of experience as a practicing pharmacist in a clinical environment.
  • Preferred experience includes working with specialty populations in a clinical setting.
