This role is for a Medication Therapy Management Pharmacist dedicated to supporting the Digestive Health Service line. The pharmacist will operate both onsite and remotely, providing essential medication management services.
Key Responsibilities
- Deliver comprehensive medication management to patients, ensuring that all medications are appropriately indicated, effective, safe, and convenient.
- Conduct thorough evaluations of patient histories and clinical data to identify, resolve, and prevent medication therapy issues.
- Collaborate with healthcare providers to make informed recommendations regarding medication therapy, adhering to established collaborative practice agreements.
- Assess the clinical significance of adverse reactions and complications related to drug therapy.
- Ensure patients are well-informed about their therapy goals and actively engage in achieving these objectives.
- Document patient outcomes at each encounter, in line with the Medication Therapy Management practice model.
- Maintain professional proficiency by engaging in continuous education through literature review and attendance at relevant seminars and conferences.
- Contribute to departmental planning and goal-setting initiatives.
- Provide mentorship and training to pharmacy students and residents, ensuring a quality experiential learning environment.
Qualifications
Education:
- Required: B.S. Degree in Pharmacy or PharmD from an accredited institution.
- Required: Current Minnesota pharmacy license and Basic Life Support Certification.
- Preferred: Ambulatory Pharmacy Board Certification (BCACP) or equivalent.
Experience:
- Minimum: PGY1 residency or three years of experience as a practicing pharmacist in a clinical environment.
- Preferred: Experience in a specialty clinic with a focus on specialty populations.
