SUMMARY:

The Minnesota Visiting Nurse Agency is seeking a Clinical Pharmacist to enhance our Inpatient Pharmacy operations. This part-time role encompasses 16 hours per pay period, featuring rotating day and evening shifts, including every fourth weekend.

Position Purpose: The Clinical Pharmacist is accountable for the accurate preparation and dispensing of medication orders, ensuring proper drug handling, usage, and storage within the pharmacy. This role upholds departmental standards and translates policies into effective pharmacy operations. The pharmacist serves as a vital information resource for patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related inquiries. Additionally, this position involves planning, organizing, and supervising the work schedules of pharmacy personnel as needed.

KEY RESPONSIBILITIES:

  • Verify, prepare, compound, and dispense medications accurately.
  • Interpret physician medication orders for accuracy, addressing any errors or discrepancies, and suggest alternative therapies based on patient evaluations.
  • Maintain precise patient medication profiles, ensuring the correct choice of drug, dosage, and monitoring for potential interactions.
  • Monitor patient responses to drug therapies.
  • Oversee the dispensing and record-keeping of controlled substances.
  • Act as a resource for medical personnel regarding medication-related inquiries.
  • Counsel patients on their prescriptions effectively.
  • Stay informed of legal requirements and departmental policies regarding drug distribution.
  • Assume charge pharmacist responsibilities as needed, coordinating the activities of pharmacy staff.
  • Conduct educational sessions for medical staff, nurses, and pharmacy personnel on drug therapies and policies.
  • Support pharmacy leadership in aligning departmental goals with organizational objectives.
  • Provide ongoing assessment of pharmacotherapy needs for patient populations, collaborating with medical and nursing staff for optimal drug therapy.
  • Critically evaluate medical information to provide accurate guidance to physicians.
  • Assist in training pharmacy technicians and interns.
  • May mentor pharmacy residents or students during their rotations.
  • Perform related duties as required.

QUALIFICATIONS:

  • Graduation from an accredited pharmacy program.

Preferred Qualifications:

  • Advanced degree or enrollment in an advanced degree program.
  • At least one year of experience as a registered pharmacist in a hospital environment.
  • Completion of an ASHP accredited residency.
  • Board certification or advanced credentials in a relevant practice area.
  • Completion of ASHP PGY-2 residency in specialized areas (e.g., critical care, infectious diseases).

LICENSE/CERTIFICATIONS:

  • Valid pharmacy license to practice in the State of Minnesota.
