Position Overview:
This role is designated for a Medication Therapy Management Pharmacist dedicated to supporting the Digestive Health Service line. The pharmacist will operate both onsite and remotely, providing comprehensive medication management services tailored to the unique needs of each patient.
Key Responsibilities:
1. Deliver Medication Therapy Management (MTM) to patients in accordance with established MTM practice models.
2. Ensure all medications prescribed are appropriate, effective, safe, and convenient for the patient.
3. Collect patient histories and integrate this information with other clinical data for thorough evaluations.
4. Identify, address, and prevent medication therapy issues.
5. Assess clinical signs and symptoms related to disease processes and recommend modifications to medication therapy based on collaborative practice agreements.
6. Evaluate the clinical significance of adverse reactions and complications linked to drug therapy.
7. Ensure patients comprehend the specific goals of their therapy and collaborate with providers to achieve these objectives.
8. Document patient outcomes during each encounter as mandated by the MTM practice model.
9. Maintain expertise in pharmacy practice by reviewing relevant literature and participating in professional development opportunities.
10. Engage actively in departmental planning, goal setting, and the execution of those plans.
11. Provide a valuable experiential learning environment for pharmacy students and residents through mentorship and training.
12. Uphold active preceptor status as required by the Minnesota Board of Pharmacy.
Qualifications:
Education:
- B.S. Degree in Pharmacy or PharmD from an accredited institution.
Licensure/Certification:
- Current Minnesota pharmacy license.
- Certification in Pharmaceutical Care as recognized by Fairview Pharmacy Services Management.
- Basic Life Support Certification.
- Preferred: Ambulatory Pharmacy Board Certification (BCACP) or equivalent.
Experience:
- Minimum: PGY1 residency or three years of experience as a practicing pharmacist in a clinical environment.
- Preferred: Experience in a specialty clinic or residency with an ambulatory focus.
