SUMMARY:

The Minnesota Visiting Nurse Agency is seeking a Clinical Pharmacist to enhance our Inpatient Pharmacy operations. This part-time role involves 16 hours per pay period, with a rotating schedule of day and evening shifts, including every fourth weekend.

Purpose of the Role: The Clinical Pharmacist is accountable for the accurate filling and dispensing of medication orders, ensuring the proper management, application, and storage of pharmaceuticals within the pharmacy. This position upholds departmental standards and translates policies into effective pharmacy operations. The Clinical Pharmacist serves as a vital information resource for patients, physicians, nurses, and other healthcare professionals regarding medication therapy and drug-related inquiries. Responsibilities also include planning, organizing, and supervising the work of pharmacy staff as necessary.

KEY RESPONSIBILITIES:

  • Verifies, prepares, compounds, and dispenses medications with precision.
  • Interprets physician medication orders for accuracy, addressing any errors or discrepancies, and suggests alternative therapies when appropriate.
  • Maintains comprehensive patient medication profiles, ensuring optimal drug selection, dosage, and monitoring for potential interactions.
  • Monitors patient responses to drug therapies.
  • Oversees the dispensing and record-keeping of controlled substances.
  • Acts as a resource for medical personnel regarding drug information.
  • Counsels patients on their prescribed medications.
  • Stays informed of legal requirements and departmental policies related to drug distribution.
  • Assumes leadership responsibilities in the absence of management, coordinating pharmacy staff and resolving issues.
  • Conducts educational sessions for healthcare staff on medications and pharmacy policies.
  • Collaborates with pharmacy supervisors and management to align departmental activities with organizational goals.
  • Provides ongoing assessments of patient pharmacotherapy needs, facilitating real-time interventions with medical and nursing staff.
  • Evaluates and interprets medical information to provide accurate guidance to physicians.
  • Supervises and trains pharmacy technicians and interns.
  • May mentor pharmacy fellows, residents, or students.
  • Performs additional related duties as required.

QUALIFICATIONS:

  • Graduation from an accredited college with a degree in Pharmacy.

Preferred Qualifications:

  • Advanced degree or enrollment in an advanced degree program.
  • At least one year of experience as a registered pharmacist in a hospital environment.
  • Completion of an ASHP accredited residency.
  • Board certification or advanced credentials in a specialized area of practice.
  • Completion of ASHP PGY-2 residency in specialized fields (e.g., critical care, infectious diseases).

Licensure:

  • Must possess a valid pharmacy license in the State of Minnesota.
