Position Title: Care Management Outreach Specialist

Location: This role will require a combination of remote and in-office work. Candidates should ideally be located within a 50-mile radius of our PulsePoint facilities.

The Care Management Outreach Specialist is tasked with establishing communication between Care Management initiatives and designated members. Operating under general oversight, this position involves managing both incoming and outgoing calls to engage eligible members and inform them about program offerings.

Key Responsibilities:

  • Collects and documents pertinent member details via phone in accordance with established protocols.
  • Promotes member involvement in Care Management initiatives by articulating the advantages, services available, and overall significance of the programs.
  • Records specifics of each call and utilizes call scripts tailored to client requirements.
  • Directs member inquiries to the appropriate clinical personnel.
  • Facilitates referrals and monitors consultations from healthcare providers for enrolled Care Management participants.
  • Gathers information from diverse sources including hospitals, health insurance plans, and medical offices.
  • Contacts physician offices for necessary demographic information when required.
  • Provides guidance and support to junior Health Program Advocates.

Essential Qualifications:

  • High school diploma or equivalent qualification.
  • A minimum of 6 months of relevant work experience; or a combination of education and experience that meets the job criteria.

Preferred Skills:

  • Availability to work from 7:00 AM to 4:00 PM PST is strongly preferred.

For individuals working on-site or remotely in designated areas, the salary range for this position is $17.78/hr. to $26.66/hr. Eligible locations include various states.
