Care Coordinator for Harm Reduction Specialist

Fair Haven Community Health Care is seeking a highly skilled and compassionate Care Coordinator for Harm Reduction to join our team. As a key member of our Harm Reduction Center, you will play a vital role in addressing the social and structural factors that impact our clients' health and well-being.
Key Responsibilities- Client Intake and Coordination: Screen clients for social determinants of health and develop individualized plans to address their needs.
- Liaison with Community Resources: Establish and maintain relationships with local organizations and agencies to link clients to essential services.
- Case Management: Work collaboratively with clinical teams to ensure clients receive comprehensive care and support.
- Documentation and Reporting: Accurately document client referrals, encounters, and services in our electronic health record system.
- Program Development and Evaluation: Assist in the development and evaluation of harm reduction programs and services.
- Education and Experience: Bachelor's degree in a health-related field and a valid CT driver's license or reliable transportation.
- Language Skills: Bilingual Spanish/English required.
- Skills and Abilities: Strong communication and interpersonal skills, ability to work in a fast-paced environment, and experience with electronic health records.
