Job Summary

We are seeking a skilled clinician to serve as the on-site clinical leader at our Harm Reduction Center. This individual will provide high-quality medical care to individuals seeking services at the Harm Reduction Center in a manner that is consistent with Fair Haven Community Health Care policies and mission.

Key Responsibilities
  • Provides primary medical care, including urgent care and primary care, for adult patients
  • Appropriately offers and prescribes medications for opioid use disorder (MOUD) and medications for alcohol use disorder (MAUD) as clinically indicated
  • Appropriately offers and prescribes pre-exposure prophylaxis (PrEP) for HIV prevention as clinically indicated
  • Develops a plan of care for each patient, including medical history, physical examination, diagnosis and appropriate treatment and/or referral, including hospitalization if necessary
  • Uses all available resources in diagnosis and treatment, such as laboratory and other tests in an appropriate and cost-efficient manner
  • Confers with other clinicians, patient's families and other support staff concerning treatment and care of patients when appropriate
  • Refers those cases which require specialist services, but maintains primary care responsibility to assure continuity of care
  • Participates in FHCHC's Quality Improvement program, in other task committees as needed, and pursues continuing education opportunities to update medical knowledge and assure quality patient care
Qualifications
  • Graduation from an accredited APRN or PA program; license to practice medicine in the state of Connecticut; ability and willingness to serve on the active medical staff of the hospitals used by FHCHC; qualified in Basic Life Support; experience prescribing MOUD, MAUD, and PrEP
  • Bilingual (English/Spanish) is preferred
