Medical Director for Harm Reduction Services

Fair Haven Community Health Care is seeking a dedicated and compassionate clinician to serve as the on-site clinical leader at our Harm Reduction Center. As a key member of our team, you will provide high-quality medical care to individuals seeking services at the Harm Reduction Center, upholding the principles of harm reduction and using a recovery-oriented approach to all patients.
Key Responsibilities- Provide primary medical care, including urgent care and primary care, for adult patients
- Offer and prescribe medications for opioid use disorder (MOUD) and medications for alcohol use disorder (MAUD) as clinically indicated
- Offer and prescribe pre-exposure prophylaxis (PrEP) for HIV prevention as clinically indicated
- Develop a plan of care for each patient, including medical history, physical examination, diagnosis, and appropriate treatment and/or referral
- Use all available resources in diagnosis and treatment, such as laboratory and other tests, in an appropriate and cost-efficient manner
- Confers with other clinicians, patient's families, and other support staff concerning treatment and care of patients when appropriate
- Refer cases that require specialist services, while maintaining primary care responsibility to ensure continuity of care
- Participate in FHCHC's Quality Improvement program and pursue continuing education opportunities to update medical knowledge and ensure quality patient care
- Graduation from an accredited APRN or PA program
- License to practice medicine in the state of Connecticut
- Ability and willingness to serve on the active medical staff of the hospitals used by FHCHC
- Qualified in Basic Life Support
- Experience prescribing MOUD, MAUD, and PrEP
- Bilingual (English/Spanish) is preferred
