Outpatient Clinical Pharmacist

POSITION OVERVIEW
The Outpatient Clinical Pharmacist works collaboratively with a diverse healthcare team to deliver comprehensive medication management, enhance medication accessibility, and provide thorough medication education to patients, all aimed at optimizing clinical outcomes.
KEY RESPONSIBILITIES
1. Implements evidence-based medication management strategies tailored to a designated patient demographic as part of a collaborative care team.
2. Engages with interdisciplinary teams to ensure optimal patient care and service delivery.
3. Instructs patients on medication usage and related health conditions, adapting communication to meet cultural and health literacy needs.
4. Guides patients on medication accessibility and adherence strategies.
5. Offers expert advice to the medical team regarding pharmacotherapy and medication-related information.
6. Aids in the refinement and documentation of clinical protocols and participates in quality improvement initiatives.
7. Ensures a safe and hygienic clinical setting for all, adhering to infection control standards and protocols.
QUALIFICATIONS
Required:
- Bachelor's degree in Pharmacy.
- Active Pharmacist license in North Carolina.
Preferred:
- Doctoral degree in Pharmacy.
- Completion of a Postgraduate Year One (PGY1) ambulatory pharmacy residency or equivalent relevant experience.
- Board Certified Pharmacotherapy Specialist (BCPS) credential from the Board of Pharmacy Specialties (BPS).
- Eligibility for Clinical Pharmacist Practitioner (CPP) licensure.
