Clinical MDS Nurse Leader

We are seeking a highly organized and efficient Registered Nurse to join our clinical leadership team as a skilled MDS Nurse at The Laurels of Hamilton.
About the RoleThis is a full-time opportunity where you will work closely with our interdisciplinary team to complete RAI assessments, care plans, and coordinate resident care. As an MDS Nurse, you will be responsible for conducting physical assessments, interviewing residents and caregivers, and reviewing chart information to identify resident needs.
Key Responsibilities- Assessment and Care Planning: Complete MDS, CAA's, and care plans within regulated timeframes, ensuring accurate and comprehensive resident assessment and care planning.
- Collaboration and Communication: Discuss resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Review and Coordination: Review information from hospitals, consults, and outside agencies, incorporating this information into the assessment and care planning process.
- Registered Nurse (RN) licensure.
- Knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment Process (PPS).
- Experience as an MDS Nurse (preferred but willing to teach).
The Laurels of Hamilton offers a competitive salary range of $80,000 - $110,000 per year, depending on experience, along with a comprehensive benefits package, including health insurance, 401K with matching funds, paid time off, and paid holidays.
