We are seeking a highly motivated and organized MDS Nurse to join our team at The Laurels of Hamilton. As a key member of our clinical leadership team, you will be responsible for ensuring the highest standards of care for our residents.

Your responsibilities will include:

  • Completing the MDS, CAA's and care plans within regulated time frames.
  • Assessing resident needs through physical assessment, interview and chart review.
  • Discussing resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
  • Reviewing information from hospital consults and outside agencies and using such information in the completion of the assessment and care planning.

We offer a competitive salary range of $60,000 - $80,000 per year, depending on experience, as well as a comprehensive benefits package, including health insurance, 401K with matching funds, paid time off and paid holidays.
