The Laurels of Hamilton offers a challenging and rewarding opportunity for an experienced Registered Nurse (RN) to join our clinical leadership team as a MDS Nurse.

We are seeking a skilled professional to work closely with our interdisciplinary team, ensuring the highest standards of care for our residents. As an MDS Nurse, you will be responsible for completing assessments and care plans within regulated time frames, discussing resident care needs with caregivers, and reviewing information from hospitals and outside agencies.

Key Responsibilities:

  • Conduct thorough assessments of residents through physical evaluation, interview, and chart review.
  • Discuss resident care needs with a multidisciplinary team, including physicians, nurses, social services, therapy, dietary, and activity staff.
  • Complete the MDS, CAA's, and care plans within regulated time frames, maintaining accurate records and communicating effectively with the team.

Requirements:

  • Registered Nurse (RN) licensure in good standing.
  • Knowledge of the Resident Assessment Instrument (RAI) process and Prospective Payment System (PPS).
  • Experience as an MDS Nurse or similar role preferred.
