Clinical Care Coordinator

The Laurels of Hamilton seeks a full-time Clinical Care Coordinator to join our clinical leadership team. This role is ideal for a Registered Nurse who wants to remain clinically involved in patient care without being a bedside nurse.
We are an 80-bed skilled nursing facility offering one of the leading employee benefit packages in the industry, including health insurance, 401K with matching funds, paid time off and paid holidays.
This position offers a competitive salary range of $60,000 - $90,000 per year, depending on experience.
Responsibilities:- Completes Resident Assessment Instruments (RAI) and care plans within regulated time frames.
- Assesses residents through physical assessment, interview, and chart review.
- Coordinates resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Reviews information from hospitals, consults, and outside agencies to inform assessments and care planning.
The successful candidate will be a detail-oriented and organized individual with excellent communication skills. They will have knowledge of the RAI process and experience as an MDS Nurse, although this is not required.
