Job Summary:
This full-time position involves working closely with our interdisciplinary team to ensure the highest standards of care for our residents. As an MDS Nurse, you will be responsible for conducting assessments and care plans, discussing resident care needs with caregivers, and reviewing information from hospitals and outside agencies.
Responsibilities:
- Conduct thorough assessments of residents through physical evaluation, interview, and chart review.
- Discuss resident care needs with a multidisciplinary team, including physicians, nurses, social services, therapy, dietary, and activity staff.
- Complete the MDS, CAA's, and care plans within regulated time frames, maintaining accurate records and communicating effectively with the team.
