Position Overview
The Advanced Practice Provider (APP), which includes Physician Assistants and Nurse Practitioners, plays a crucial role within the acute care hospital environment. This position involves collaboration with a diverse healthcare team to deliver comprehensive inpatient care.

Responsibilities
  • Assessment & Management: Conduct thorough history and physical examinations, documenting findings accurately in the medical record. Utilize appropriate assessment techniques and relevant diagnostic information to inform patient care. Act as a primary responder for patient status changes and emergencies, executing necessary interventions. Evaluate risks associated with managing acute and complex chronic conditions, including medication effects and nutritional status.
  • Diagnosis: Work alongside the interdisciplinary healthcare team to establish diagnoses for acute and chronic conditions. Oversee diagnostic testing, including ordering and interpreting results, and prioritize differential diagnoses. Identify complications and initiate appropriate interventions.
  • Care Planning: Develop and document a comprehensive plan of care based on expected outcomes from diagnoses, addressing complex health needs. Employ evidence-based guidelines to create a dynamic and individualized care plan. Adjust and implement care strategies as necessary, including prescribing diagnostic and therapeutic interventions.
  • Communication & Collaboration: Maintain continuous communication with the healthcare team, ensuring effective collaboration. Engage with patients and their families to facilitate continuity of care across inpatient and outpatient settings.
  • Documentation: Accurately document patient progress through daily notes, transfer summaries, and discharge documentation, ensuring compliance with acute care standards and capturing patient acuity.
  • Professional Development: Exhibit professional behaviors through mentoring and educating graduate students. Seek opportunities for involvement in research and evidence-based practice analysis. Participate in relevant professional meetings and maintain continuing education requirements.

Qualifications
  • Master's Degree in a relevant field.
  • National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant.
  • BLS certification is required.
  • EPIC EMR training is preferred.
