Inpatient Care Advanced Practice Provider

The Advanced Practice Provider (APP), comprising both Physician Assistants and Nurse Practitioners, plays a crucial role within the acute care hospital environment, working synergistically as a vital component of a multidisciplinary healthcare team. The APP is responsible for delivering comprehensive medical care on the inpatient hospitalist service.
Assessment & Management
- Conducts thorough history and physical examinations, ensuring accurate documentation in the medical records.
- Utilizes appropriate assessment techniques to order and gather relevant diagnostic information and procedures as necessary.
- Acts as the primary responder for nursing and house staff during patient status changes, orders, and critical situations requiring procedural and resuscitative actions.
- Evaluates risks associated with the management of acute and complex chronically ill patients, including medication side effects, immobility, nutritional impairment, fluid and electrolyte imbalances, immunocompetence, and invasive interventions.
- Maintains an average daily census of patients.
- Collaborates with the interdisciplinary healthcare team to establish diagnoses for acute and complex chronic conditions.
- Oversees diagnostic testing through ordering, interpretation, execution, and supervision.
- Develops differential diagnoses based on priority.
- Identifies complications and prescribes appropriate interventions.
- Determines expected outcomes from diagnoses and formulates a documented care plan addressing complex acute and chronic healthcare needs.
- Employs evidence-based practice guidelines and protocols to create a dynamic, individualized care plan applicable across the continuum of care.
- Implements and adjusts the care plan as necessary.
- Prescribes diagnostic and therapeutic interventions.
- May perform advanced procedures within the scope of privileges and competency validation.
- Ensures continuous communication and collaboration with the interdisciplinary healthcare team.
- Facilitates effective communication with patients, families, and staff to enhance continuity of care across inpatient and outpatient settings.
- Records essential components of patient progress through daily notes, transfer summaries, discharge summaries, and clinic notes as applicable.
- Ensures documentation is timely, adheres to acute care compliance standards, and accurately reflects patient acuity.
- Exhibits professional practice behaviors, including mentoring and educating graduate students.
- Seeks opportunities for active participation in research and the evaluation of evidence-based practices.
- Engages in Advanced Practice Grand Rounds and specialty-specific meetings.
- Maintains continuing education units (CEUs) and membership in a professional organization.
- Possesses a Master’s Degree.
- Holds National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant.
- Is certified in Basic Life Support (BLS).
- EPIC EMR training is preferred.
