Job Description

Job Summary

We are seeking a skilled Patient Care Coordinator to join our team at Option Companion Care. In this role, you will work collaboratively with patients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client receives high-quality, individualized care.

The ideal candidate is compassionate, patient, and knowledgeable about healthcare practices. Key responsibilities include:

  • Providing Coordinated Care: Developing, monitoring, and evaluating patient home health care plans to ensure seamless care delivery.
  • Ensuring High-Quality Care: Overseeing the delivery of care by home health aides to ensure patients receive the highest level of care.
  • Scheduling and Coverage: Scheduling agency home health care aides and ensuring they work their scheduled shifts, with the ability to manage last-minute call-outs and find suitable coverage.
  • Communication and Customer Service: Communicating effectively with referral sources and Service Coordinators to provide an exceptional customer service experience.
  • Patient Education and Matching: Educating patients on their healthcare options and matching them with the most suitable caregiver(s).
  • Caseload Management: Managing a caseload of 100+ patients, ensuring timely and effective care delivery.
  • Customer and Patient Service: Providing great customer and patient service, with the ability to work under pressure.
  • Review and Resolution: Reviewing care plans with patients and caregivers, resolving grievances and complaints, and addressing overutilization of hours.
  • Reporting and Performance: Reporting personnel performance issues and maintaining detailed records.
  • Team Collaboration: Contributing to team efforts by accomplishing related results as needed, recruiting and training staff.

Qualifications

  • Education: High school graduate
  • Experience: 3 years of experience in a Home Care or Service Coordination role
  • Confidentiality: Demonstrated capability to maintain strict confidentiality
  • Leads and Referrals: Proven ability to generate leads and monitor referrals
  • Technical Skills: Strong typing and computer skills, comfortable with closing and asking for business
  • Organization and Attention to Detail: Well-organized, accurate, and attentive to detail
  • Communication and Public Relations: Excellent communication, public relations, and follow-up skills
  • Experience with HHA Exchange and EVV: Experience with HHA Exchange and EVV
