Position Overview

The Advanced Practice Provider (APP), encompassing both Physician Assistants and Nurse Practitioners, plays a pivotal role within the acute care hospital environment, working synergistically as a vital member of a multidisciplinary healthcare team. The APP is responsible for delivering comprehensive medical care on the inpatient hospitalist service.

Assessment & Management
  • Conducts thorough history and physical examinations, meticulously documenting findings in the medical record.
  • Orders and gathers relevant data utilizing appropriate assessment techniques and diagnostic procedures as necessary.
  • Acts as the primary responder for nursing and house staff during patient status changes, managing orders and executing procedural and resuscitative interventions.
  • Evaluates risks associated with the care of acutely and chronically ill patients, considering factors such as medication side effects, immobility, impaired nutrition, and more.
  • Maintains an average daily census of patients.
Diagnosis
  • Collaborates with the interdisciplinary healthcare team to establish diagnoses for acute and complex chronic conditions.
  • Oversees diagnostic testing through ordering, interpretation, and performance.
  • Develops differential diagnoses based on priority.
  • Identifies complications and prescribes appropriate interventions.
Plan of Care Development
  • Determines expected outcomes from diagnoses and formulates a comprehensive plan of care addressing complex health needs.
  • Employs evidence-based practice guidelines to create a dynamic, individualized plan of care applicable across the continuum.
  • Implements and adjusts the plan of care as necessary.
  • Prescribes diagnostic strategies and therapeutic interventions.
  • May perform advanced procedures in accordance with established privileges and competency validation.
Communication & Collaboration
  • Ensures continuous communication and collaboration with the interdisciplinary healthcare team.
  • Facilitates discussions with patients, families, and staff to enhance continuity of care across inpatient and outpatient settings.
Documentation
  • Records essential components of patient progress through daily notes, transfer summaries, discharge summaries, and clinic notes as applicable.
  • Ensures documentation is timely, compliant with acute care standards, and accurately reflects patient acuity.
Professional Development
  • Exhibits professional practice behaviors, including mentoring and educating graduate students.
  • Seeks opportunities for engagement in research and analysis of evidence-based practices.
  • Actively participates in Advanced Practice Grand Rounds and specialty-specific meetings.
  • Maintains continuing education units (CEUs) and membership in a professional organization.
  • Holds a Master’s Degree.
  • Possesses National Board Certification as an Acute Care Nurse Practitioner or Physician Assistant.
  • Holds BLS certification.
  • EPIC EMR training is preferred.
