Position Overview
The Arbors at Dracut is seeking a dedicated PRN/PerDiem Massachusetts Licensed LPN or RN to fulfill the role of Wellness Nurse.
The Wellness Nurse is responsible for orchestrating the service plan tailored to the health requirements of residents, fostering wellness through the provision of superior services that align with the company’s Mission, Vision, and Philosophy while adhering to relevant state laws and regulations.
This role also involves supporting the Resident Care Director in overseeing and guiding the Resident Care Aides.

Key Responsibilities:
- Assist in leading and mentoring the Resident Care Aides in the execution of the company’s mission, vision, goals, and objectives.
- Support compliance with all applicable state and federal regulations.
- Ensure the delivery of exceptional resident care.
- Evaluate resident concerns and medical issues as necessary, coordinating with other service providers to guarantee appropriate care.
- Maintain precise and comprehensive reports, data, and records.
- Accurately assess, observe, document, and communicate any significant changes in resident health and behavior to the Resident Care Director, family/designated representatives, and other healthcare professionals.
- Act as a liaison between families/designated representatives, healthcare agencies, emergency response teams, hospitals, and rehabilitation facilities.
- Direct, supervise, mentor, and guide the daily tasks of the Resident Care Aides.
- Provide first aid as needed.
- Collaborate with team members to ensure quality care and services for all residents.
- Assume on-call responsibilities as required.
Qualifications:
Applicants must possess a current, valid Registered or Practical Nursing License in the state of application, along with a solid understanding of aging and disability issues.
