As a Licensed Practical Nurse, you will play a crucial role in ensuring the health, safety, and overall well-being of individuals in our group home setting. You will be responsible for managing and delivering essential medical care to our program participants, including direct nursing interventions.
Work Schedule:
Sunday: 8:00am - 9:30pm
Monday: 2:30pm - 10:30pm
Tuesday: 3:00pm - 11:00pm
Wednesday: 12:30pm - 2:30pm (bi-weekly staff meeting)
Wednesday: 2:30pm - 11:00pm
Total of 39 Hours
Key Responsibilities:
- Assess and monitor the health status of program participants, providing expert guidance on nursing care and medical treatment as necessary.
- Manage and document all medical information, referrals, reports, medication administration, and data collection.
- Conduct medical record audits and review tracking systems to ensure compliance and accuracy.
- Oversee the self-medication assessment process and provide necessary support.
- Supervise the administration of medical treatments and ensure adherence to prescribed protocols.
- Train and orient staff on understanding the medical needs of participants and following established care plans.
- Lead and support the team during medical emergencies, ensuring effective communication with families, clinicians, and other stakeholders.
- Provide guidance on medical emergencies and promptly inform supervisors of any critical incidents.
- Maintain communication with healthcare providers regarding significant health issues.
- Document nursing encounters and maintain accurate nursing notes.
- Process human rights issues and incident reports in accordance with agency policies, safeguarding the rights of all participants.
- Adhere to all agency, departmental, program, and site policies, as well as collective bargaining agreements.
Qualifications:
- Must be a Licensed Practical Nurse registered with the MA Board of Registration in Nursing.
- Minimum of 1 year of experience working with elderly individuals or those with developmental disabilities.
- Excellent interpersonal, written, and verbal communication skills are essential.
- Valid driver's license and reliable transportation are required.
