About the Role

Aveanna Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of healthcare professionals. As a LPN, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

Key Responsibilities
  • Deliver and document skilled nursing care to patients in accordance with physician orders and developed care plans.
  • Conduct thorough patient assessments and develop individualized care plans to meet their unique needs.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
Requirements
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Graduate of an accredited school of nursing.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
