Patient Care Assistant

POSITION SUMMARY:
The Patient Care Assistant supports individuals in fulfilling fundamental human requirements and daily living activities under the supervision of a Registered Nurse (RN) or Licensed Practical Nurse (LPN). This role also involves additional responsibilities to improve the coordination and collaboration of patient care as directed by the RN or LPN.
#Maintains clear and effective communication with patients, families, and colleagues at all times, while assisting with various tasks to ensure a safe and hygienic environment. Responsible for keeping accurate medical records and reporting to the Clinical Nurse Manager.
# EDUCATION:
CERTIFICATION:
High School diploma or equivalent is required. Successful completion of a certified nursing assistant program is preferred but not mandatory. Alternatively, an active Certified Nurse Assistant certification is required upon hire.
# Completion of classroom training on Nurse Aide competencies and written verification of such competencies in the clinical setting is required during the new employee orientation period, with ongoing clinical competency maintained throughout employment with ECHN in this capacity. Certification in Basic Cardiac Life Support is required upon hire or must be obtained during the new employee orientation period, with clinical competency maintained throughout employment.

Must possess the ability to read and communicate effectively in English.
Basic computer skills are essential.

#Provides general patient care that addresses physical and/or psychosocial needs within the nursing assistant's scope of practice. Assists with the admission, transfer, and discharge processes of patients.
Demonstrates the ability to communicate any observed changes in patient status and responses to the RN or LPN. Notifies appropriate licensed personnel when patients express discomfort. Collects, reports, and assists in documenting relevant data.
Performs all aspects of patient care in an environment that prioritizes patient safety and promotes positive outcomes. Exhibits flexibility, organization, and the ability to function effectively under pressure. Communicates clearly and appropriately with team leaders, coworkers, and other healthcare team members.
Undertakes additional duties as assigned to facilitate the efficient operation of the unit. Safely transports patients to other departments and during admission and discharge as necessary. Maintains clear communication with charge nurses, coworkers, and other members of the healthcare team.
Performs duties as trained, including but not limited to the removal of saline locks, Foley catheters, pulse oximetry, emptying Jackson-Pratt drains, telemetry application, EKGs, disconnecting/reconnecting Salem sump, glucose meter usage, measuring and recording nasogastric drainage, and other assigned tasks.
