About Our Team

At Hospice & Palliative Care, we pride ourselves on being a collective of healthcare professionals devoted to delivering compassionate and high-quality end-of-life care. Our Registered Nurses (RNs) play a crucial role in supporting patients with life-limiting conditions and their families, ensuring that care is respectful, dignified, and tailored to individual needs.

Key Responsibilities of the Registered Nurse:

  • Manage a reasonable caseload within the community.
  • Continuously communicate patient care updates and progress to the attending physician and care team.
  • Oversee and guide the care provided by Certified Home Health Aides (CHHAs) and Licensed Practical Nurses (LPNs).
  • Collaborate with patients and families to develop and implement personalized care plans aimed at addressing their unique needs.
  • Maintain accurate medical documentation for all patients, including case conferences, patient interactions, medication adjustments, re-certifications, progress notes, and care plan modifications.
  • Deliver hands-on care, manage care plans, and educate patients in accordance with physician directives, adjusting plans as necessary based on ongoing assessments.
  • Provide comprehensive admission, case management, and follow-up nursing visits for hospice patients as required.
  • Engage in bi-weekly team meetings and case discussions to ensure optimal patient care and service delivery.
  • Foster a collaborative and supportive team environment.
  • Contribute to the provision of quality, comprehensive services through a team-oriented approach.
  • Perform additional duties as assigned.

Qualifications and Experience:

  • Current RN licensure in the state of New York is mandatory.
  • Possession of a valid New York State Driver's License and automobile insurance.
  • A minimum of two years of nursing experience in settings such as hospitals, nursing homes, oncology, home health agencies, hospice, or long-term care facilities is preferred.

Skills and Competencies:

A genuine passion for hospice care is essential. Candidates must possess strong assessment, evaluation, and communication skills. Demonstrated knowledge and sound judgment in patient care and decision-making are required. CPR certification is necessary, and additional medical skills are highly valued. Participation in ongoing education to enhance knowledge and skills relevant to job performance is expected. Reliable transportation and a valid driver's license are required.
