Job Summary

We are seeking a highly skilled and compassionate Resident Services Coordinator to join our team at 2Life Communities. As a key member of our staff, you will play a vital role in providing support and services to our residents, ensuring they have the resources and care they need to thrive in our supportive housing communities.

Key Responsibilities
  • Conduct Resident Assessments: Periodically assess residents' social, psychological, cognitive, and physical status, and develop personalized plans to address their needs.
  • Advocate for Residents: Advocate on behalf of residents for timely and cost-effective provision of services, ensuring they receive the support they require.
  • Participate in Integration Initiatives: Collaborate with our Housing and Health Care Integration team to develop and implement initiatives that promote the well-being of our residents.
  • Respond to Resident Emergencies: Respond promptly to resident emergency calls, working closely with other staff members to ensure timely and effective support.
  • Provide Education and Support: Educate, assist, and advise residents and their families on available services and resources, both within our organization and in the community.
  • Pursue Additional Services: Identify and pursue opportunities for additional services through private, local, state, and federal sources, enhancing the quality of life for our residents.
  • Document Resident Interactions: Maintain confidential files of all significant contacts with residents, social service providers, medical providers, families, and for HUD reporting purposes.
  • Coordinate Care Transitions: Coordinate care across medical providers for residents, ensuring safe transitions in discharge planning with residents, families, and hospital or rehab teams.
Requirements
  • Education and Certification: Master's or Bachelor's degree in Social Work, Gerontology, or a related field, with Resident Services Coordinator Certification or willingness to attain within the first year of employment.
  • Experience and Skills: Proven experience working with older adults, general computer literacy with Microsoft Office products and database software, and bilingual proficiency in Russian.
