Responsibilities:
  • Screen patients via phone and in-person inquiries, verifying insurance coverage and scheduling assessments and intakes.
  • Complete all necessary paperwork accurately and efficiently, adhering to company policies and procedures.
  • Collaborate effectively with hospital staff, including physicians and nurses, to ensure seamless patient care and discharge planning.
  • Maintain confidentiality and adhere to HIPAA guidelines at all times.
  • Conduct daily facility rounds and follow-up calls to patients, as per company policy and procedure.
  • Develop personalized discharge and aftercare plans in consultation with patients.
  • Report daily census to the corporate office.
  • Engage in community outreach efforts, making daily telephone calls to various agencies.
  • Conduct bio-psychosocial assessments.
  • Perform other related duties as assigned by supervisor.
Qualifications:
  • Bachelor's degree in Human Services or a related field, with a minimum of 2 years of experience in social services and/or clinical work.
  • Knowledge of addiction and/or dependence is essential.
  • Previous experience as a substance abuse counselor or case manager is highly desirable.
  • Ability to work effectively as part of a team, with minimal supervision, and to support colleagues in achieving shared goals.
  • Strong problem-solving skills and ability to handle stressful situations.
  • Prior experience in a hospital setting and hospital management is preferred.
  • Satisfactory drug screen and criminal background check results.
