Job Title: Detox Intake Coordinator

Special Care Hospital Management is seeking a highly skilled and compassionate Detox Intake Coordinator to join our team. As a key member of our medical staff, you will be responsible for the screening and assessment of patients to ensure they meet our service criteria. Your expertise will also be utilized in developing patient-tailored discharge plans for those who have completed treatment.

Responsibilities:
  • Conduct thorough screenings and assessments of patients via phone and in-person inquiries, ensuring accurate insurance verification and scheduling of assessments and intakes.
  • Prepare and complete all necessary paperwork in a clear, concise, and policy-compliant manner.
  • Establish effective communication with hospital staff, including physicians and nursing personnel, regarding patient assessment, continued stay, and discharge planning.
  • Maintain confidentiality and adhere to HIPAA guidelines.
  • Conduct daily facility rounds and follow-up calls on patients, adhering to established policies and procedures.
  • Collaborate with patients to formulate discharge and aftercare plans.
  • Provide daily census reports to the corporate office.
  • Engage in community outreach telephone calls to various agencies.
  • Complete bio-psychosocial assessments.
  • Perform other related duties as assigned by your supervisor.
Qualifications:
  • Bachelor's degree in Human Services or a related field, with a minimum of 2 years of experience in social services and/or clinical work.
  • Knowledge of addiction and/or dependence is essential.
  • Previous experience as a substance abuse counselor or case manager is a plus, particularly in substance abuse withdrawal, MAT treatment, or detox.
  • Ability to work collaboratively as part of a team, with minimal supervision, and provide support to colleagues.
  • Strong problem-solving skills and ability to implement solutions.
  • Capacity to handle stressful situations and interact effectively with others.
  • Prior experience in a hospital setting and hospital management is preferred.
  • Satisfactory drug screen and criminal background check.
