Job Summary

Lantern Community Services Inc is seeking an experienced Case Manager to join our team at our men's shelter, Laurel Hall. As a Case Manager, you will be part of an interdisciplinary team aimed at stabilizing shelter clients by improving their independent living skills and assisting clients in moving toward their housing goals.

Key Responsibilities
  • Assess client needs and develop, monitor, and evaluate treatment plans and progress.
  • Admit new clients by reviewing records and applications, conducting orientations, and determining mental and medical needs.
  • Establish treatment programs by setting schedules and routines, coordinating services, arranging resources, including transportation and escort.
  • Monitor cases by verifying clients' attendance, observing and evaluating treatments and responses, advocating for needed services and entitlements, obtaining additional resources, intervening in crises, and providing personal support.
  • Communicate clients' progress by conducting weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to the therapeutic team and family, identifying treatment influences.
Requirements
  • High school diploma, HSE or GED, required.
  • A minimum of 2 years of social services experience required.
  • Experience working with the homeless, mentally ill, substance abuse, and/or low-income populations.
  • Strong relationship-building skills are required.
  • Strong Computer skills required including Word and Excel.
  • Ability to work Evenings and/or Weekends on a rotating schedule.
  • Preferred experience working in a homeless shelter.
  • Knowledge of mental health, substance use, or homeless.
  • Bilingual (English/Spanish) preferred.
Work Environment

This job operates in a shelter environment in Queens, NY, and works with a population who may suffer from mental illness, substance abuse, disability, HIV/AIDS, and/or other health issues.

Physical Demands

While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally the employee will have to lift and/or move up to 10 pounds.
