Role Summary

As an integral member of the healthcare team, the Patient Care Coordinator:

  • Ensures that the care delivered by the healthcare professionals is executed in an efficient and cost-effective manner, while striving for optimal patient outcomes.
  • Assists the physician and healthcare team in advocating for the judicious use of healthcare resources.
  • Guarantees that patients are admitted to the appropriate level of care, ensuring timely service through standardized criteria and adherence to federal and state regulations.
  • Collaborates with patients, caregivers, and the healthcare team to evaluate the discharge requirements of the patient, developing and facilitating a safe and suitable discharge plan.

Minimum Qualifications

Education:
Graduate of an accredited nursing program is required. A Bachelor of Science in Nursing (BSN) is preferred.

Experience:
3-5 years of experience in acute care or community nursing is required.

Licensure and Certification:
Current licensure as a registered nurse in Massachusetts is required. Certification in Case Management (CCM) is preferred.
